Schedule: Full-time Monday through Friday.
Responsibilities: In this emotionally rewarding role, you will be building relationships with facilities and case managers to discharge patients to Brightstar care services as they transition to in-home care. You will have regular contact with patients and referral sources. Must work well in a team environment.
